Fees

• Initial Intake Appointment $225

• Individual Therapy $200

• Couples or Parent/Child $225

• Aetna

• United Healthcare / Optum

• Missouri Medicaid

• Medicare

 

I accept cash, check, Venmo 

 

Those who choose to pay privately have the benefit of not having diagnostic and treatment information shared with their insurance providers, and having treatment dictated or restricted by insurers.

• Payment must be provided at the time of service

• There will be $40 for a returned check

• I can provided a super bill for insurance

 

Out of Network

If I am not in-network with your insurance plan, many of my clients are able to receive partial reimbursement from their insurance policies via out-of-network benefits. I am able to provide you with a document called a superbill which is essentially a receipt containing the codes that insurance companies use to process claims. Please check with your insurance policy to verify coverage and details for out-of-network mental health benefits.

 

I am an out of network provider for all other insurances. Many insurance plans offer out of network mental health care benefits, and will reimburse you for a percentage of the cost of our sessions. Here are some questions that you may ask your insurance company:

• Do I have out-of-network benefits for mental health coverage?

• If so, what percentage is covered?

• Is there a session limit, and if so, in what time period?

• Do I have a deductible? What is it and how much of it have I met?

• How do I submit a request for reimbursement?

• What is the reimbursement for CPT code 90791? 90834? 90837?

 

Cancellation Policy

Dr. Nelson requires 48 hours notice for appointment changes or cancellations. Late cancellations will result in a $60 charge for the scheduled session. In cases of illness, blizzard, or other such circumstances Dr. Nelson may waive the late cancellation fee, though this will be at Dr. Nelson’s full discretion. Please note that insurance plans do not reimburse clients for fees resulting from late cancellations or missed sessions. Patient will be responsible for payment.
